Stock photography and model release forms

Question

I am planning on shooting some "lifestyle" images with a model and was wondering about the type of model release that would be required. Would a "generic" do or does it have to be a "specific" one. What I mean is the form says something along the lines of;

 

"I (model name) authorise the Photographer to use the images as he sees fit"

 

or

 

"I (model name) authorise the photographer to use the images lifestyle use"

 

Not posted much (due to imposter syndrome) so looking for guidance

I use my wife in a lot of shots, no payment involved, would I need to fill that box in or just put zero?

Pay her a very small sum or take her out to dinner and divide the cost as payment over however many photos.   You're not likely to get sued by your wife,are you?  My understanding is that payment makes a contract valid, even if only one peppercorn.
