Certainly with some publications like text books images may be included in an early draft but then rejected or replaced by the time the book is published in its final form. With traditional photo agencies (where Alamy has its roots) it is normal business practice for images which don't actually make the final publication to be refunded as 'unused'. This approach may also be the case for other types of publication, e.g. websites produced by a professional designer where the client insists on changes to particular images before the website is signed off.
